The Origin Story

2025 was one of the hardest years our family has ever faced. Within months, we found ourselves caring for two loved ones; one battling advanced-stage cancer, the other newly diagnosed with dementia. Overnight, our roles shifted in a way we never expected: we went from being children to being primary caregivers.

What we encountered was overwhelming. It took months just to piece together which providers were involved, what medications were prescribed, and what diagnoses and allergies existed. Along the way, we uncovered dangerous medication overlaps and significant overmedication; clear signs that communication across providers was breaking down.

Like many families, we turned to a binder to centralize everything. And while it helped, it was far from perfect. There was only one physical copy. It constantly needed updates. Multiple caregivers lived in different places. And unless you were holding the binder, you had no access to critical information. Keeping everyone informed became a daily struggle.

 

‘Which test was ordered, and did it get scheduled?’ 

'What bills need to be paid?' 

‘Did Dad take his meds?’ 

‘Who’s taking Mom to chemo?’

We were spending so much time trying to stay coordinated that it pulled us away from the moments we wanted most: being present with our parents during the time they needed us.

 

We knew there had to be a better way. But the tools available (Excel trackers, patient portals, scattered notes) were fragmented and not built for families like ours navigating complex care. When we couldn’t find the solution we desperately needed, we decided to build it.

 

That’s how Your CareMap was born.

 

Our mission is simple: to give individuals, families, and caregivers the clarity, tools, and support they need to coordinate care, track medications, and make informed decisions with confidence. With chronic conditions rising and our population aging, caregiver-focused solutions are more important than ever.
