How many individuals can I track care for with my subscription to Your CareMap?
Each subscription to Your CareMap is designed to coordinate the care and records for one individual. Add Family and Caregivers as users to update and coordinate care.
How many users can I add to coordinate care in Your CareMap?
There is no limit to the number of users at this time.
Will the “Care Team” (Providers, Social workers, Spiritual Care, etc.) be able to access the records in Your CareMap?
Only if you add them as users. Your CareMap is designed to help individuals, families and caregivers navigate complex care. It is not intended for professional use.
Can I use insurance to pay for the subscription to Your CareMap?
No. YourCareMap, LLC (“Your CareMap”) is not a healthcare provider, physician, or medical practice.
